Travel Consent Form

I declare to know and understand the risk involved
participating in these activities where the risk cannot be completely eliminated, even if there is
compliance with safety standards approved by the provider which aim to reduce the risk that the
development of this activity involves.

NOTE: It is the duty of the travel agency to report the conditions and requirements for the development
of the activity, as it is also the duty of participants to take on the service adequately informed of the
conditions under which the service is developed, the minimum conditions that the participant must have
prior to the activity and comply with the instructions that the guide and company in charge of the
activity indicates.
